AERIAL WORK PLATFORM DAILY PRE-SHIFT INSPECTION

Before equipment operation on each work shift, the aerial work platform and area of planned usage shall be
given a visual inspection by the operator for equipment defects and the location of hazards that could affect the
equipment’s safe operation and use. SAT = Satisfactory UNSAT = Unsatisfactory N / A = Not Applicable

7. EQUIPMENT INSPECTION – Operating and emergency controls
8. Safety Devices and designed fall protection tie-off points
9. Fuel, hydraulic and batter water levels
10. Air, fuel and hydraulic lines
11. Cables and wiring harnesses
12. Associated equipment parts and assemblies (check for damage)
13. Tires and wheels (proper inflation and condition)
14. Equipment welds (check for any cracked welds)
15. Structural members (check for bent or broken members)
16. Outriggers, stabilizers, extendible axles and other relevant components
17. Warning and control decals or placards (present and legible)
18. Guardrail system, safety gates and chains
19. Platform condition (platform is free of slip and trip hazards)
20. Operating and Safety Manual available & legible
21. Correct Personal Protective Equipment in place (PPE)
22. SITE INSPECTION – Ditches or trenches
23. Drop-offs, slopes, and extreme elevation changes
24. Holes
25. Bumps, floor obstructions or protrusions
26. Overhead power lines or other electrical conductors
27. Wind and inclement weather
28. Other equipment or personnel affecting operation of aerial lift
29. Sufficient clearance
30. Surface capable of supporting the equipment in all configurations
